The NC Foothills Affiliate of Susan G. Komen for the Cure established the Kathy Kent Volunteer for the Cure Award in memory of Katharine Harper Kent. Kathy was instrumental in the implementation of the NC Foothills Affiliate serving Burke, Caldwell and Catawba Counties. She was an inspiration to many. Because of her commitment to volunteerism and her dedication to finding a cure for breast cancer, the Affiliate will award the “Kathy Kent Volunteer for the Cure" Award annually to the most outstanding volunteer.

Eligibility Requirements:

1. Nominees can be from any field: academia, volunteer or business. Nominees must be affiliated with the local Affiliate. NC Foothills Affiliate Board members are not eligible for the award
   
   
2. Nominees must demonstrate the following:
   
   A commitment to the fight against breast cancer and to the NC Foothills Affiliate through volunteer work, donations, support of Komen-related events, raising awareness in the community, etc.
   
   An outstanding contribution to the philosophy of the Komen mission/vision.
   
   Application of knowledge/education in practical ways; viewed as a valuable source of experience and expertise; advice and counsel sought by others pertaining to the Komen mission.
   
   
3. A volunteer may receive the award only once.
   
   
4. A volunteer who was nominated but did not receive the award may be nominated during subsequent years.
   

Selection/Presentation Process:

The Kathy Kent Volunteer for the Cure Award recipient will be selected by a judging committee composed of 4-6 members including a member of the Affiliate’s Board of Directors. The recipient will receive the Affiliate's signature print, framed, with an engraved name plate. The current recipient will present the award at the Affiliate’s Komen Race for the cure® in October to the new recipient.

Nomination Form Instructions:

1. Nomination forms should be typed in 10-12 pt. font or printed.
   
   
2. No additional pages may be added to the nomination form.
   
   
3. Answers must be provided on the front of the form only. Back of pages may not be used.
   
   
4. Each nomination may include one Komen-related photograph of the nominee.
   
   
5. Completed forms should be submitted to:
